As of March 13, 2026, CMS has released an updated Advanced Beneficiary Notice (ABN). The revised form carries a date of March 31, 2029, and should be used going forward. Please replace and discard the previous version dated January 31, 2026; note that the older form remains acceptable through May 12, 2026. The updated form can be found on the CMS website.
This form applies to traditional (fee-for-service, Part B) Medicare only and is not appropriate for Medicare Advantage plans or other payers. For most other insurance types and health plans, a Notice of Non-Coverage is the correct form to use.
