It is a common story—theory and practice are never quite the same. The challenges and time constraints of clinical practice require clinicians to modify practices to increase efficiency. Most of these adaptations are based on proper application of theory; however, inconsistencies among audiologists can lead to confusion. In this article, several clinical scenarios are presented to highlight some common variances seen in practice. We assume that proper testing procedures have been followed in all scenarios. The difficulty comes in the observed inconsistencies of recording thresholds among clinics and even among audiologists within the same facility. Our traditional symbols (TABLE 1) convey not just thresholds but also how test results were acquired.
