This guide aims to provide coding, billing, and coverage guidance for otoacoustic emissions.

Clinicians are strongly encouraged to reference payer-specific policies when determining appropriate code reporting and documentation requirements for testing. Further, Medicare local coverage determinations and other payer policies will address which ICD codes support medical necessity and subsequent reimbursement for services. It is advisable to review payer guidelines and policies prior to billing.

Relevant Procedure Codes

The following Level I HCPCS CPT codes are available for reporting assessment using otoacoustic emissions.

CPT CODEDescription
92558Evoked otoacoustic emissions, screening (qualitative measurement of distortion product or transient evoked otoacoustic emissions), automated analysis
92587Distortion product OAEs (DPOAEs) or transient evoked OAEs (TEOAEs), limited evaluation (to confirm the presence or absence of hearing disorder, 3-6 frequencies), with interpretation and report
92588Comprehensive diagnostic evaluation (quantitative analysis of outer hair cell function by cochlear mapping, minimum of 12 frequencies), with interpretation of the test, with a report *(CPT Manual Instructions: Do not report in conjunction with 92620 or 92621.)
*Note: The CPT manual parentheticals presented here do not include Medicare’s National Correct Coding Initiative (NCCI) procedure to procedure edits. The full list of NCCI edits and other information related to same-day billing can be found here.

Additional Information and FAQ

Otoacoustic emissions can be a critical component to the comprehensive audiologic examination.  Like all tests, otoacoustic emissions are not warranted in every test scenario, so documentation must substantiate the need for service. Additionally, codes that include the language “with interpretation and report,” must include interpretation in the patient’s medical record. A printout from the equipment by itself is not considered a report.
